• I understand that to the extent our work together involves career or business, my Personal Success Mentor is not promising outcomes included but not limited to increased clientele, profitability and or business success. Business mentoring is not a substitute for engaging the services of a CPA, financial services advisor/specialist or Attorney.

• I understand that my Personal Success Mentor will protect my information as confidential unless I state otherwise in writing. If I report child, elder abuse or neglect or threaten to harm myself or someone else, I understand that necessary actions will be taken and my confidentiality agreement limited in this capacity.

• I understand that any decisions I make, and the consequences thereof are my own. This service is requested at my own choice and with inherent singular responsibility. • I take full responsibility in the decisions I make after being mentored as well as the consequences. I enter into mentoring with full understanding that I am responsible for creating my own results.
